Hello, I used to work for Express permits, which is the section of the department where we'd issue out temporary power pole permits, so I can only give information on that side of the process. Depending on whether you are obtaining the permit via owner builder or contractor, there are several documents you need to complete in order to obtain a temporary power permit. Those documents will depend on who's applying for the permit but generally, an electrical application and signature declaration will be needed.
If it's owner-builder, additional paperwork will include the owner builder declaration. Of course, that is if it's owner-builder applicable. If going the contractor route, a C10 (electrician) would need the application and signature declaration. Be sure to have the document signed by the contractor on the state licensing board. A notarized letter will be needed if someone else signs the document.
